**Answer:** A supporter is an adult you choose to help make decisions about your care, according to your wishes, if you become unable to decide for yourself. You appoint them through a written supportive decision-making agreement, signed when you understand what you are doing and witnessed by two people, one of whom is your doctor. A supporter must act in line with your will and preferences.
