# What documents must support an SHA claim?

**Answer:** SHA's contract lists the beneficiary verification reference, any pre-authorisation reference, the itemised invoice coded to the tariff, and clinical records for the service line. For admissions this includes the admission and discharge record, treatment sheet and medication chart, investigation results and a discharge summary signed by the treating professional. Claims must use SHA's prescribed diagnostic coding (the claim form asks for ICD-11 codes) and the patient or authorised person signs a declaration. SHA can ask for supporting documents within seven days, so keep them retrievable by claim number.
