**Answer:** Pain assessment in physiotherapy looks at more than intensity. Clinicians record location, quality, timing, triggers, easing factors, irritability, effect on sleep and function, beliefs, mood, medication and warning signs. Scales such as the Numeric Pain Rating Scale (usually 0 to 10) or the Visual Analogue Scale (a mark on a continuous line) help track change over time for the same person. A score alone does not show the cause or seriousness of pain, so it is interpreted alongside function and examination.

### NPRS versus VAS

- NPRS uses discrete numbers and is quick to use; specify current, average, best or worst pain and the time period
- VAS uses a continuous line; vision, cognition, language, motor control and screen size can affect completion
- Digital versions are fine if validated and displayed consistently
- Scores are most reliable for tracking one person over time, not comparing people; children may need alternative scales

Pain can be present without visible tissue damage. Reassess pain when treatment changes or symptoms worsen. A low score does not rule out serious pathology; severe new pain with red flags needs escalation.
