**Answer:** Discharge from physiotherapy does not always mean full recovery. It may mean you can now manage safely on your own, or that another service is a better fit. Before discharge, your physiotherapist should review your progress, remaining limitations, exercise plan, equipment needs and warning signs, and explain when to seek help. You should leave knowing what to keep doing, how to progress it and who to contact. You can usually return if symptoms come back, function declines or you have new goals.

### What a good discharge plan includes

Your goals, current function, outcome measures, exercise technique, equipment, work or sport demands and any outstanding referrals should be reviewed first.

- An exercise schedule and rules for progressing it
- Warning signs that mean you should seek help
- Follow-up arrangements and how to come back
- Guidance on returning to work, sport or other activities
- Information for other providers involved in your care

Discharge should not happen simply because sessions have run out if serious symptoms, unsafe mobility or unmet care needs remain.
