> A simple five-line structure for therapy session notes: what to record, what to leave out, and how practices in Kenya can write clear notes in minutes, not hours.

A good session note is short. Five lines can cover most sessions: why the client came today, what you noticed, what you did, how it went against the goal, and what happens next. Write it straight after the session, use the same structure every time and keep it in one secure system. The structure does the thinking about format, so you can focus on the content.

## Why do therapy notes take so long to write?

Notes take long because each one starts from a blank page. Many therapists decide the format again for every session, then worry they have missed something.

In our conversations with counsellors and psychologists in Kenya, note-writing was the task they named most often. They described turning an hour of therapy into a few useful lines as the hardest part of the admin week.

The time adds up. A 2019 survey of 499 therapists found that a note took about eight minutes on average, and 22% said each note took ten minutes or more. With 20 clients a week, ten minutes a note is more than three hours.

A fixed structure cuts most of that time. When every note has the same five headings, you only decide what goes under each one.

## What are the five lines of a clear session note?

The five lines follow the order of the session. Each answers one question.

1. Reason for today: what the client brought to the session, in their own words where useful.
2. What you noticed: presentation, engagement and any measure completed, such as a PHQ-9 score.
3. What you did: the approach or intervention you used.
4. Progress against the goal: one sentence linking the session to the treatment plan.
5. Next steps: homework, referrals, any follow-up and the date of the next session.

If you already use SOAP, the five lines fit inside it. Lines one and two cover Subjective and Objective, line four is your Assessment, and lines three and five form the Plan. Our guide to [SOAP notes for therapy](https://tymirahealth.com/resources/blog/soap-notes-for-therapy-a-practical-guide-for-mental-health-clinics) explains each section.

Where a session raises a risk concern, add a short line on what you observed and the action you took under your clinic's policy.

## What does a five-line note look like?

Here is a fictional example for a client working on stress at work.

1. Reason for today: client said work deadlines had kept her awake most nights this week.
2. What you noticed: arrived on time, engaged well, spoke quickly when describing her manager. GAD-7 score 12, down from 15 two weeks ago.
3. What you did: reviewed last week's sleep diary and practised a breathing exercise for use before bed.
4. Progress against the goal: some progress towards the goal of sleeping six hours on most nights.
5. Next steps: continue the sleep diary and the breathing exercise daily. Next session in one week.

That note is five lines long. A colleague or supervisor can read it in under a minute and know what changed.

## What should you leave out of a session note?

Leave out anything that does not help care, continuity or accountability. A note is a record, not a transcript.

- Long verbatim quotes, unless the exact words matter for care.
- Details about other people that are not relevant to the client's care.
- Guesses and opinions that are not part of your professional assessment.
- Labels or loaded language about the client. Describe what you saw instead.
- Repeated history that is already in the intake record.

Shorter notes are also safer. The less sensitive detail a record holds, the less harm a breach can do.

## How can a practice keep notes consistent across clinicians?

Consistency comes from shared rules, not individual habits. Agree them once and write them down.

- Use one template for every clinician in the practice.
- Agree standard wording for common goals and interventions.
- Choose the same measures and record them the same way.
- Set a time limit for completing notes, such as the same day.
- Review a sample of notes in supervision each month.

## Where should session notes be kept in Kenya?

Session notes should be kept in one secure system with access limited by role. Under Kenya's Data Protection Act 2019, health data is sensitive personal data.

That rules out notes on personal WhatsApp chats, shared laptops or open paper files. Reception and finance staff need appointment and billing details, not session content. See our guide to [role-based access for therapy notes](https://tymirahealth.com/resources/blog/role-based-access-for-therapy-notes-and-mental-health-records).

Tymira360 gives each session a structured note template linked to the client's goals and assessment scores. Clinicians still decide what to record. The software keeps the format consistent and the record secure.

## Key takeaways

- Most sessions can be recorded in five lines.
- Write the note straight after the session, while it is fresh.
- Use the same structure for every client and every clinician.
- Leave out transcripts, guesses and details that do not help care.
- Keep notes in one secure system with access limited by role.

## Frequently asked questions

**How long should a therapy session note be?**

Long enough to show what happened, what you did and what comes next. For most sessions, five short lines are enough.

**When should I write my session notes?**

As soon as possible after the session, ideally within the hour. Notes written straight away are quicker to write and more accurate.

**Does the five-line structure replace SOAP or DAP?**

No. It fits inside them. The five lines are a quick way to fill whichever format your practice already uses.

**Should I record test scores in the note?**

Yes, if the client completed a measure in the session. Record the score and compare it with the last one.

**Can clients see their session notes?**

Under the Data Protection Act 2019, people have a right to access their personal data. Agree a clear process for handling requests with your clinical lead.

This article is for operational education. Note templates and clinical workflows should be reviewed by a qualified clinical lead before use.
